Affidavit for a Child’s Passport to be issued in India

Currently WALK-IN services are SUSPENDED. Please send requisite document by mail along with all supporting documents (notarized copies of passport/legal status/address proof) and return shipping envelope. PLEASE DO NOT SEND ORIGINAL PASSPORT.  

The Consulate reserves the right to reject attestation of documents the contents of which are objectionable or contrary to the Rules.

The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India ) are required to be presented to authorities in USA, these should be apostilled in India. It is mandatory for the documents to be attested to be apostiled by the relevant authorities in the USA for getting the documents attested at the Consulate.

Cashier's Check/Money Order should be drawn in favour of CONSULATE GENERAL OF INDIA, SAN FRANCISCO" NO BLANK CHECK/MONEY ORDER WILL BE ACCEPTED BY THE CONSULATE

Supporting Documents: Photocopy of all supporting documents should be notarized and attached to the application.

Fee:  A non-refundable fee of $ 20 (twenty) + $2 Indian Community Welfare Fee (ICWF).

Fees should be paid through Money order or Cashiers’ cheque drawn in favor of the ‘Consulate General of India, San Francisco’NO BLANK CHECK/MONEY ORDER WILL BE ACCEPTED BY THE CONSULATE. Cash, Personal Cheques, Credit Cards, Debit Cards or other banking instruments are not accepted.

Please enclose a trackable pre-paid self-addressed return mailing envelope (USPS 'Express Mail' or UPS etc.) (No Fedex) for dispatching the processed documents.

How to Apply:

  • Miscellaneous Services Form- Download the Miscellaneous Application form  and complete the application form.
  • A Notarized copy of the affidavit in the prescribed format to be submitted in duplicate along with self-attested copy of the birth certificate of child.
  • Attach the following documents with the application form.
  • Notarized copy of Passport (First and Last Page)
  • Proof of your US Visa Status:(Notarized copy of any one of the following)
    • Photocopy of the page containing visa on passport (H1B, H4 etc) and the copy of I-94
    • Clear Photocopy of Green Card
    • Employment Authorization Document (Work Permit)
    • I-797, I-140 or I-20 (If approval copy of these notices are pending, also attach a handwritten note detailing the efforts being taken to regularize status)
  • Proof of current US residence address: (Notarized copy of any one of the following)
    • S Driving license.
    • PG&E, Water or landline telephone bill displaying applicant’s address
    • House Lease Agreement
    • State Identification Card
    • Note:Bank/credit card/mobile phone statements are not accepted as residence proof
  • In case the Consulate receives applications with incomplete documentation, the same would be sent back unprocessed in the return envelope sent by the applicant.
