About Us Guidance On Passport Services: Supporting Document

Irrespective of the nature of service, the following minimum of documents should invariably be submitted for all passport, miscellaneous and PIO card services :

Please note that for all cases where the police verification status is incomplete, Personal Verification Form is to be submitted. In order to reduce the processing time, it is advisable that the applicant also include filled in Personal Verification Form  while sending their application.

All mail applications submitted to the Consulate for consular services with a non-trackable return mailing envelope will be returned to the applicant without services. Please ensure to send a trackable return mailing envelope (e.g. UPS, USPS, etc..) to ensure traceability of documents after dispatch.

To enable us properly scrutinize, coloured photocopy of your documents is preferred

    • All previous passports in original, current passport in original and photocopy of the first five, last two pages, and of the page containing visa of the current passport
    • Proof of Status:
      • Clear colored Photocopy of Green Card, Employment Authorization Document or other visa documents
      • If application for Green Card/EAD/Visa is pending/being processed, document should be submitted
    • Proof of Residence:
      Note: Bank/credit card/mobile phone statements are not accepted as residence proof
      • Driving license or
      • PG&E, Water or landline telephone bill displaying applicant’s address or
      • House Lease Agreement or
    • Two (2) photographs of 2 inches x 2 inches or 35mm x 35mm photos - one to be pasted in the box on page 1 (one) and the 2nd photo to be stapled on the blank portion (below ‘for official use only’) on page 4 (four).
    • When applying in person, please carry originals of all the supporting documents, photocopies of which you are enclosing with the application. The originals have to be presented to the Consular Officer
  • If applying by mail, Photocopy of all supporting documents including Photo should be notarized and attached to the application. If original supporting documents are enclosed, there is no need of notarization. When the original passport is enclosed, there is no need to notarize photocopy
  • Requirement of any additional documents are referred to under specific services.
